Your first visit to Foot and Ankle Center of Lake City establishes a vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know Dr. Berg. Your comfort during your visit is important to us.

    Your FBS (Fasting Blood Sugar last taken by your Primary Care Physician)

About Our Office and Parking For Your Appointment
The Foot and Ankle Center of Lake City is located just west of the Lake City Library in a historic building, formerly Lake City Elementary School. The exterior looks much like it did, "in the day," with brick walls and the original, tall, wood-framed windows. The inside has been transformed into a charming medical and office building which houses our practice along with physical therapy, dental, and chiropractic services.

Several parking spaces are reserved at the front of the building for patients of the Foot and Ankle Center of Lake City. Our office, Suite 130, is just a few steps away. Wheelchair access and additional free parking are located at the east and west sides of the building.

Make an Appointment
Call 206-368-7000 during regular business hours to schedule an appointment or if you need an appointment right away. If your condition is an emergency, call 9-1-1 or go to the emergency room at your nearest hospital.

You can request an appointment from our website at any time. Our staff will confirm you appointment by phone as soon as possible. If your request is arrived after regular hours, we will contact you the next business day.

If for any reason you cannot keep a scheduled appointment, or will be delayed, please call us as soon as possible.

Insurance
Our office accepts a variety of HMOs, PPOs, and other health plans. Please call our office to verify acceptance of your insurance carrier.

From the South
Take I-5 north and Exit at 130th St. Make a right turn onto 130th. This street will change names several times and eventually will become 125th. Travel for about 1 mile. Once you pass 26th Ave NE, the Lake City Professional Center will be on your right. Pass the building and turn into the parking lot. We have designated parking right in front. Wheelchair accessible parking is in the back.

From the North
Take I-5 south and exit at 145th St. Turn L at 145th and continue until 15th Ave NE. Turn R and travel until you reach 125th St. Turn left and drive for about a mile. Once you pass 26th Ave NE, the Lake City Professional Center will be on your right. Pass the building and turn into the parking lot. We have designated parking right in front. Wheelchair accessible parking is in the back.
